ABSTRACT

ABSTRACT Purpose: Contrast-enhanced CT scan is the standard staging modality for patients with bladder cancer undergoing radical cystectomy (RC). Involvement of lymph nodes (LN) determines prognosis of patients with bladder cancer. The detection of LN metastasis by CT scan is still insufficient. Therefore, we investigated various CT scan characteristics to predict lymph node ratio (LNR) and its impact on survival. Also, pre-operative CT scan characteristics might hold potential to risk stratify cN+ patients. Materials and Methods: We analyzed preoperative CT scans of patients undergoing RC in a tertiary high volume center. Retrospectively, local tumor stage and LN characteristics such as size, morphology (MLN) and number of loco-regional LN (NLN) were investigated and correlation to LNR and survival was analyzed. CT scan characteristics were used to develop a risk stratification using Kaplan-Maier and multivariate analysis. Results: 764 cN0 and 166 cN+ patients with complete follow-up and imaging data were included in the study. Accuracy to detect LN metastasis and locally advanced tumor stage in CT scan was 72% and 62%. LN larger than 15mm in diameter were significantly associated with higher LNR (p=0.002). Increased NLN correlated with decreased CSS and OS (p=0.001: p=0.002). Furthermore, CT scan based scoring system precisely differentiates low-risk and high-risk profiles to predict oncological outcome (p <0.001). Conclusion: In our study, solely LN size >15mm significantly correlated with higher LNR. Identification of increased loco-regional LN was associated with worse survival. For the first time, precise risk stratification based on computed-tomography findings was developed to predict oncological outcome for clinical lymph node-positive patients undergoing RC.

ABSTRACT Introduction: The therapeutic role of pelvic lymph node dissection (PLND) in prostate cancer (PCa) is unknown due to absence of randomized trials. Objective: to present a critical review on the therapeutic benefits of PLND in high risk localized PCa patients. Materials and Methods: A search of the literature on PLND was performed using PubMed, Cochrane, and Medline database. Articles obtained regarding diagnostic imaging and sentinel lymph node dissection, PLND extension, impact of PLND on survival, PLND in node positive "only" disease and PLND surgical risks were critically reviewed. Results: High-risk PCa commonly develops metastases. In these patients, the possibility of presenting lymph node disease is high. Thus, extended PLND during radical prostatectomy may be recommended in selected patients with localized high-risk PCa for both accurate staging and therapeutic intent. Although recent advances in detecting patients with lymph node involvement (LNI) with novel imaging and sentinel node dissection, extended PLND continues to be the most accurate method to stage lymph node disease, which may be related to the number of nodes removed. However, extended PLND increases surgical time, with potential impact on perioperative complications, hospital length of stay, rehospitalization and healthcare costs. Controversy persists on its therapeutic benefit, particularly in patients with high node burden. Conclusion: The impact of PLND on biochemical recurrence and PCa survival is unclear yet. Selection of patients may benefit from extended PLND but the challenge remains to identify them accurately. Only prospective randomized study would answer the precise role of PLND in high-risk pelvis confined PCa patients.

ABSTRACT

Objective: To evaluate the feasibility of identification and preservation of arm lymphatics (DEPART) in axillary lymph node dissection (ALND) for breast cancer to prevent arm lymphedema. Methods: A randomized controlled study method was used. Two hundred and sixty-five patients who underwent breast cancer surgery at the Department of Thyroid and Breast Surgery, Zhongnan Hospital of Wuhan University from November 2017 to June 2018 were included, and the patients were randomly divided into ALND+ DEPART group (132 patients) and standard ALND group (133 patients) by random number table method. In the ALND+ DEPART group, indocyanine green and methylene blue were injected as tracers before surgery, and the arm sentinel nodes was visualized by staged tracing during intraoperative dissection of axillary lymph nodes. Partial frozen sections were made of arm lymph nodes >1 cm in length and hard and suspicious of metastasis, and arm lymph nodes and lymphatic vessels were selectively preserved. Patients in the standard ALND group underwent standard ALND. Objective and subjective indexes of arm lymphedema were evaluated by 5-point circumference measurement and Norman questionnaire. Results: Among 132 breast cancer patients in the ALND+ DEPART group, 121 (91.7%) completed DEPART. There were no statistically significant differences in age, body mass index, pathological type, dissection number of axillary lymph node, N stage, TNM stage, molecular typing, and regional radiotherapy between the ALND+ DEPART and standard ALND groups (P>0.05). At a median follow-up of 24 months, assessment by the 5-point circumference measurement showed that the incidence rates of lymphedema in the ALND+ DEPART and standard ALND groups were 5.0% (6/121) and 15.8% (21/133), respectively, with statistically significant differences (P=0.005). Assessment by the Norman questionnaire showed that the incidence rates of lymphedema in the ALND+ DEPART and standard ALND groups were 5.8% (7/121) and 21.8% (29/133), respectively, with a statistically significant difference (P<0.001). No local regional recurrence was observed in either group during the follow-up period. Conclusion: For breast cancer patients with positive axillary lymph nodes, the administration of DEPART during ALND can reduce or avoid the occurrence of arm lymphedema without compromising oncology safety.

ABSTRACT

Cervical cancer is the most common gynecological malignant carcinoma in Chinese women, which seriously threatens women's health. Lymph node metastasis is the most important factor affecting the prognosis and adjuvant treatment of patients with early cervical cancer. Conventional systematic lymphadenectomy has always been a reliable method to detect lymph node metastasis. However, this procedure may affect the quality of life of patients because of high risks of complications. Recently, sentinel lymph node biopsy (SLNB) has been researched aiming to assess the status of pelvic lymph node metastasis intraoperatively, avoid unnecessary lymphadenectomy and achieve the goal of individualized diagnosis and treatment. Exploring efficient tracing methods and pathological ultra-staging is the key to the clinical application of SLNB for cervical cancer, and understanding the latest relevant clinical research progress will help SLNB to be applied in the clinic as soon as possible to benefit patients with cervical cancer.

ABSTRACT

Objective: To investigate the relationship between the examined number of lymph nodes at the N1 station and the clinicopathological characteristics and prognosis of patients with pT1-3N0M0 non-small cell lung cancer (NSCLC). Methods: A total of 337 patients with pT1-3N0M0 NSCLC who underwent radical lung cancer surgery at the Provincial Hospital Affiliated to Anhui Medical University from January 2013 to March 2015 were selected. The receiver operating characteristic (ROC) curve analysis was used to determine the optimal cut-off value for predicting 5-year survival in pT1-3N0M0 NSCLC patients by the examined number of lymph nodes at the N1 station. The relationships between the examined number of lymph nodes at the N1 station and the clinicopathological characteristics and prognosis of patients with pT1-3N0M0 NSCLC were analyzed according to the optimal cut-off group. Results: A total of 1 321 lymph nodes at N1 station were examined in 337 patients, with a mean of 3.9 nodes per patient. The median survival time was 42.0 months, with 1-, 3- and 5-year survival rates of 82.2%, 57.1% and 24.9%, respectively. ROC curve analysis showed that the optimal cut-off value of 4.5 lymph nodes examined at the N1 station was used to predict 5-year survival in patients with pT1-3N0M0 NSCLC. After rounding off the number, the number of lymph nodes examined at the N1 station was 5 as the cut-off value, and the patients were divided into the group with <5 lymph nodes examined (212 cases) and the group with ≥5 lymph nodes examined (125 cases). The proportion of patients received adjuvant chemotherapy was 19.2% in the group with ≥5 lymph nodes examined, which was higher than 9.0% in the group with <5 lymph nodes examined (P=0.007), and the differences in other clinicopathological characteristics between the two groups were not statistically significant (P>0.05). The median survival time for patients in the group with <5 lymph nodes examined was 38.0 months, with 1-, 3- and 5-year survival rates of 80.1%, 52.5% and 15.6%, respectively. The median survival time for patients in the group with ≥5 lymph nodes examined was 48.0 months, and the 1-, 3- and 5-year survival rates were 85.6%, 64.0% and 36.0%, respectively. The survival rate of patients in the group with ≥5 lymph nodes examined was better than that in the group with <5 lymph nodes examined (P=0.002). Multifactorial Cox regression analysis showed that T stage (OR=1.408, 95% CI: 1.118-1.670) and the examined number of lymph nodes at N1 station (OR=0.670, 95% CI: 0.526-0.853) were independent influence factors for the prognosis of pT1-3N0M0 NSCLC patients. Conclusion: The examined number of lymph nodes at the N1 station is associated with the prognosis of patients with pT1-3N0M0 NSCLC, and the examination of at least 5 lymph nodes at N1 station at the time of postoperative pathological examination improves the 5-year survival rate of patients.

ABSTRACT

Esophageal cancer is one of the most common malignant tumors of digestive tract, lymph node metastasis is a frequently encountered metastasis in the esophageal cancer patients. The number of lymph node metastasis is reported as an important prognostic factor, and it also affects the choice of postoperative treatments in the esophageal cancer. It was reported that the recurrent laryngeal nerve lymph nodes are the most common sites of nodal metastasis and need to be completely dissected during the esophagectomy for thoracic esophageal cancers. Dissection of the lymph nodes along bilateral recurrent laryngeal nerves not only improves the accuracy of staging, but also improves postoperative survival of esophageal cancer patients due to reducing the local recurrence. However, it also brings problems such as injury of laryngeal recurrent nerves, and increases postoperative complications such as pulmonary complications and malnutrition due to aspiration and coughing. Therefore, it is necessary to preserve the structure and function of bilateral recurrent laryngeal nerves during esophagectomy through careful manipulations, and minimize the impact of complications in prognosis and quality of life from injury to the recurrent laryngeal nerve.

ABSTRACT

Standardized surgical management of postoperative specimens of gastric cancer is an important part of the standardized diagnosis and treatment of gastric cancer. It can reflect the accurate number and detailed distribution of lymph nodes in the specimen and lay the foundation for accurate and standardized pathological reports after surgery. Meanwhile, it can evaluate the scope of intraoperative lymph node dissection, the safety of cutting edge, and the standardization of surgery (principle of en-bloc dissection), which is an important means of surgical quality control. It also provides accurate research samples for further research and is an important way for young surgeons to train their clinical skills. The surgical management of postoperative specimens for gastric cancer needs to be standardized, including specimen processing personnel, processing flow, resection margin examination, lymph node sorting, measurement after specimen dissection, storage of biological specimens, documentation of recorded data, etc. The promotion of standardized surgical management of specimens after radical gastrectomy can promote the homogenization of gastric cancer surgical diagnosis and treatment in medical institutions and further promote the high-quality development of gastric cancer surgery in China.

ABSTRACT

The proportion of adenocarcinoma of the esophagogastric junction (AEG) in gastric cancer is gradually increasing. Due to the unique anatomical structure and biological characteristics of the tumor at this site, AEG has a certain degree of complexity in many aspects of diagnosis and treatment, which brings difficulties to the operation method, the selection of the resection range, the lymph node dissection and the treatment decision-making. Therefore, AEG has always been the focus of academic debate. With the development of minimally invasive surgery in recent years, laparoscopic technology has been increasingly mature and widely used in the treatment of gastrointestinal tumors. Compared with distal gastric cancer, the minimally invasive treatment of AEG is in a lagging state, and there are also a series of problems that have not yet reached a consensus. This article reviews and summarizes the recent research progress in two aspects: proximal gastrectomy for AEG and lymph node dissection. Laparoscopic-assisted proximal gastrectomy is safe for early proximal gastric cancer and has a long-term survival outcome not inferior to total gastrectomy, but the surgical indications must be strictly selected. Abdominal lymph node metastasis of AEG is mainly in group 1, 2, 3, and 7, and mediastinal lymph node metastasis is closely related to the length of the infiltrated esophagus. The abdominal transhiatal (TH) approach can obtain a sufficient number of harvested lymph node, and has good safety and efficacy, which is the first-choice of surgical approach for early AEG. The results of the CLASS-10 clinical trial can provide a higher level of evidence for laparoscopic mediastinal lymph node dissection. Laparoscopic surgery for AEG should be carried out in experienced medical center based on clinical research.

Abstract Introduction Lymph node metastasis is a well-known prognostic factor for laryngeal carcinoma. However, current nodal staging systems provide limited information regarding prognosis. Additional parameters should be considered to improve prognostic capacity. Objectives To assess the prognostic values of metastatic lymph node number, ipsilateral/contralateral harvested lymph nodes, and lymph node ratio in patients undergoing surgical treatment of laryngeal squamous cell carcinoma. Methods Seventy-four patients diagnosed with laryngeal squamous cell carcinoma primarily managed surgically were included in this study. The patients' pathological and survival data were obtained from their medical records. The effects of harvested lymph nodes and lymph node ratio on disease-free survival, disease-specific survival, and overall survival were analyzed. Results Ipsilateral, contralateral, and bilateral evaluations of harvested lymph nodes showed no significant associations with prognosis. Lymph node ratio was significantly associated with overall survival when evaluated bilaterally. Metastatic lymph node number showed more suitable stratification than TNM classification. Conclusions Metastatic lymph node number and bilateral lymph node ratio parameters should be taken into consideration to improve the prognostic capacity of TNM.


Resumo Introdução A metástase linfonodal é um fator prognóstico bem conhecido para o carcinoma de laringe. Entretanto, os sistemas atuais de estadiamento nodal fornecem informações limitadas sobre o prognóstico. Parâmetros adicionais devem ser considerados para melhorar a capacidade prognóstica. Objetivos Avaliar os valores prognósticos do número de linfonodos metastáticos, linfonodos ipsilaterais /contralaterais coletados e relação de linfonodos em pacientes submetidos ao tratamento cirúrgico do carcinoma espinocelular da laringe. Método Foram incluídos neste estudo 75 pacientes com diagnóstico de carcinoma espinocelular da laringe, tratados primariamente por meio de cirurgia. Os dados histopatológicos e de sobrevida dos pacientes foram obtidos de seus prontuários médicos. Foram analisados os efeitos dos linfonodos coletados e da relação de linfonodos na sobrevida livre de doença, sobrevida doença-específica e sobrevida global. Resultados As avaliações ipsilateral, contralateral e bilateral dos linfonodos coletados não mostraram associações significativas com o prognóstico. A relação de linfonodos foi significantemente associada à sobrevida global quando avaliada bilateralmente. O número de linfonodos metastáticos mostrou estratificação mais adequada do que a classificação TNM-N. Conclusões Os parâmetros número de linfonodos metastáticos e relação de linfonodos bilateral devem ser levados em consideração para melhorar a capacidade prognóstica da classificação TNM.

Las metástasis del carcinoma papilar de tiroides (CPT) generalmente son a nivel locorregional, la diseminación a distancia es poco habitual, sin embargo la invasión de tejidos blandos aunque inusual puede ocurrir, y afecta negativamente la supervivencia. El presente estudio describe una serie de casos de Metástasis Musculares de CPT. Se realizó un estudio transversal de un solo centro que evaluó diez pacientes con CPT con metástasis en músculo. La edad de los pacientes fue entre 46 a 77 años, siendo la edad promedio de 60 años, 7 de los cuales fueron de sexo masculino que corresponde al 70%, todos con antecedente de CPT con respuesta estructural incompleta, además de las metástasis en músculo presentaron afectación de tres o más órganos, con necesidad de varios tratamientos, cada paciente registró entre 1 a 8 cirugías, recibieron entre 100 a 780mCi de I131 (yodo radiactivo), ocho ameritaron radioterapia, todos tuvieron indicación de tratamiento con ITK, sin embargo solo cuatro pacientes tuvieron acceso a dicho medicamento. La mayoría de las metástasis del CPT en músculo fueron diagnosticadas en los estudios de imagen PET/ CT, después de la tiroidectomía el tiempo de su presentación fue muy variable entre 1 a 18 años, el número de músculos comprometidos se reporta entre uno a cuatro, siendo el glúteo (4 casos) el músculo metastásico más frecuente. La presencia de metástasis musculares empeora el pronóstico en nuestra serie de pacientes.


Metastases of thyroid papillary carcinoma (CPT) are generally at the locoregional level, the dissemination from a distance is unusual, however the invasion of soft tissues, although rare can occur, and it negatively affects survival. The present study describes several Muscular Metastases of CPT cases. A transversal study in one only center was performed and assessed ten patients CPT metastases in muscles.The patients age ranged from 46 to 77, being the average age of 60, and 7 of them were male, corresponding to the 70%, everyone with CPT records with an incomplete structural response. Besides muscular metastases they also presented issues with three or more organs, needing many treatments. Each patient registered between 1 to 8 surgeries, they received between 100 to 780mCi of I131. Eight required radiotherapies, everyone required treatment with ITK, however, just four patients had access to that medication. Most of the CPT metastases in muscles were diagnosed in PET/CT image studies, after the thyroidectomy, the time for its presentation was very variable between 1 to 18 years, the number of compromised muscles is reported between one to four, being the buttock (4 cases) the most frequently muscle with metastases. The presence of muscular metastases aggravates the prognosis in our series of patients.

ABSTRACT

Cervical cancer is a common gynecologic malignancy. Most patients with early-stage cervical cancer received unnecessary systemic pelvic lymphadenectomy, which increased the risk of surgical complications. At present, sentinel lymph node biopsy has been applied in the clinical practice of cervical cancer abroad, however it is still at the starting stage in China in need of application and promotion. The Obstetrics and Gynecology Committee of Chinese Research Hospital Association invited domestic experts in the field of gynecologic oncology to discuss the application value, patient evaluation, technical methods, operation steps, pathological examination and many other key points of sentinel lymph node biopsy based on the current research status, and reached the consensus of clinical application on sentinel lymph node biopsy in cervical cancer to guide the standardized application of the technique in China.

Introducción: Los linfomas no Hodgkin (LNH) forman un grupo heterogéneo de neoplasias linfoides. Actualmente se reporta un aumento global de su incidencia. Estos se originan generalmente en los ganglios linfáticos, aunque pueden aparecer fuera de este, entre los que se describe el linfoma primario del sistema nervioso central, que puede afectar la médula espinal, entre otros. Objetivo: Presentar un paciente con diagnóstico de linfoma no Hodgkin con invasión mielorradicular, condición reportada con muy poca frecuencia. Caso clínico: Paciente de 47 años de edad con historia anterior de salud que comienza con neuralgia intercostal, progresivamente aparecen otros síntomas y signos neurológicos con deterioro de la función motora, se diagnostica linfoma no Hodgkin primario del Sistema Nervioso Central, se le realiza tratamiento y obtiene mejoría de los síntomas y control de la enfermedad. Conclusiones: Con el diagnóstico y tratamiento precoz de las lesiones compresivas del Sistema Nervioso Central, entre estas el LNH se puede lograr el control de la enfermedad y garantizar una adecuada calidad de vida con mejor pronóstico(AU)


Introduction: Non-Hodgkin lymphomas form a heterogeneous group of lymphoid neoplasms; a global increase in its incidence is currently reported. These usually originate in the lymph nodes, although they may appear outside the lymph node, among which primary lymphoma of the central nervous system, which can affect the spinal cord, is described, among others. Objective: To present a patient with a diagnosis of non-Hodgkin lymphoma with myelodradicular invasion, a condition reported very infrequently. Clinical case: A 47-year-old patient with a previous history of health that begins with intercostal neuralgia, other neurological symptoms and signs with impaired motor function appear progressively, primary non-Hodgkin lymphoma of the Central Nervous System is diagnosed, treatment is performed and Obtain improvement of symptoms and disease control. Conclusions: With the diagnosis and early treatment of the compressive lesions of the Central Nervous System, among these the NHL can control the disease and guarantee an adequate quality of life with a better prognosis(AU)

ABSTRACT Objective: Lung cancer (LC) is one of the leading causes of death worldwide. Accurate mediastinal staging is mandatory in order to assess prognosis and to select patients for surgical treatment. EBUS-TBNA is a minimally invasive procedure that allows sampling of mediastinal lymph nodes (LNs). Some studies have suggested that EBUS-TBNA is preferable to surgical mediastinoscopy for mediastinal staging of LC. The objective of this systematic review and meta-analysis was to compare EBUS-TBNA and mediastinoscopy in terms of their effectiveness for mediastinal LN staging in potentially operable non-small cell lung cancer (NSCLC). Methods: This was a systematic review and meta-analysis, in which we searched various databases. We included studies comparing the accuracy of EBUS-TBNA with that of mediastinoscopy for mediastinal LN staging in patients with NSCLC. In the meta-analysis, we calculated sensitivity, specificity, positive likelihood ratios, and negative likelihood ratios. We also analyzed the risk difference for the reported complications associated with each procedure. Results: The search identified 4,201 articles, 5 of which (with a combined total of 532 patients) were selected for inclusion in the meta-analysis. There were no statistically significant differences between EBUS-TBNA and mediastinoscopy in terms of the sensitivity (81% vs. 75%), specificity (100% for both), positive likelihood ratio (101.03 vs. 95.70), or negative likelihood ratio (0.21 vs. 0.23). The area under the summary ROC curve was 0.9881 and 0.9895 for EBUS-TBNA and mediastinoscopy, respectively. Although the number of complications was higher for mediastinoscopy, the difference was not significant (risk difference: −0.03; 95% CI: −0.07 to 0.01; I2 = 76%). Conclusions: EBUS-TBNA and mediastinoscopy produced similar results for mediastinal staging of NSCLC. EBUS-TBNA can be the procedure of first choice for LN staging in patients with NSCLC.


RESUMO Objetivo: O câncer de pulmão (CP) é uma das principais causas de morte no mundo. Um estadiamento mediastinal preciso é obrigatório para avaliação do prognóstico e seleção de pacientes para tratamento cirúrgico. EBUS-TBNA é um procedimento minimamente invasivo que permite a amostragem de linfonodos mediastinais. Alguns estudos sugerem que a EBUS-TBNA é preferível que a mediastinoscopia cirúrgica no estadiamento mediastinal do CP. O objetivo desta revisão sistemática e meta-análise foi comparar a eficácia da EBUS-TBNA e da mediastinoscopia no estadiamento linfonodal mediastinal do câncer de pulmão de células não pequenas (CPCNP) potencialmente operável. Métodos: Foram pesquisados diversos bancos de dados. Estudos comparando a precisão da EBUS-TBNA e da mediastinoscopia no estadiamento linfonodal mediastinal em pacientes com CPCNP foram incluídos. Na meta-análise, foram calculadas sensibilidade e especificidade, bem como razões de verossimilhança positiva e negativa. A diferença de risco de complicações relatadas para cada procedimento também foi analisada. Resultados: A pesquisa identificou 4.201 artigos, dos quais 5 foram selecionados para a meta-análise (total combinado de 532 pacientes). Não houve diferenças estatisticamente significativas entre EBUS-TBNA e mediastinoscopia: sensibilidade (81% vs. 75%), especificidade (100% para ambas), razão de verossimilhança positiva (101,03 vs. 95,70) e razão de verossimilhança negativa (0,21 vs. 0,23). A área sob a curva summary ROC para EBUS-TBNA e para mediastinoscopia foi de 0,9881 e 0,9895, respectivamente. Embora o número de complicações tenha sido maior para mediastinoscopia, não foi encontrada diferença significativa (diferença de risco: −0,03; IC95%: −0,07 to 0,01; I2 = 76%). Conclusões: EBUS-TBNA e mediastinoscopia apresentaram resultados semelhantes no estadiamento mediastinal do CPCNP. EBUS-TBNA pode ser o procedimento de primeira escolha no estadiamento linfonodal em pacientes com CPCNP.

ABSTRACT Objective: Our aim was to present our experiences related to performing neck surgery using the guided intraoperative scintigraphic tumor targeting (GOSTT) procedure for patients who had locally recurrent or persistent differentiated thyroid cancer (DTC) and who had undergone previous thyroid surgery. Subjects and methods: We retrospectively evaluated 11 patients who had locally recurrent or persistent DTC, who had undergone previous surgery, and for whom reoperation was planned for metastatic cervical lymph nodes (LNs). We performed the neck surgery using the GOSTT procedure on all patients and at a single academic institution. Results: The 11 patients had a total of 26 LNs, as marked with a radiotracer, and those LNs' mean size was 14.7 ± 8.2 mm (range: 5-34 mm). Histopathological examinations revealed DTC metastasis in all 26 of the preoperatively marked LNs. Of the 11 patients, only one needed a reoperation in the neck; she had another successful surgery (also using the GOSTT procedure). In the evaluation of the patients' final status, all were disease-free in their necks. There also were no GOSTT-associated postoperative complications. Conclusion: The GOSTT procedure is a useful, successful, inexpensive, and comfortable procedure for marking and mapping metastatic LNs, especially in DTC patients who have undergone previous surgery.

Introducción: El cáncer de pene tiene una alta incidencia en Latinoamérica, siendo el carcinoma epidermoide el tipo histológico más frecuente. También es sabido que el cáncer de pene suele presentar diseminación a los ganglios linfáticos inguinales, antes de la progresión a la enfermedad metastásica. Objetivo: Caracterizar a los pacientes tratados por cáncer de pene con invasión locorregional en el Servicio de Urología del Hospital Central del Instituto de Previsión Social (HCIPS) de Asunción del Paraguay. Materiales y métodos: Se realizó un estudio retrospectivo, de pacientes tratados con cáncer de pene con invasión locorregional entre los años 2014 y 2017. Se analizaron las siguientes variables: estadificación, tratamientos empleados según la clasificación y complicaciones. Resultados: Los 18 pacientes tratados por cáncer de pene con invasión locorregional fueron adultos mayores, con una edad media de 78,5 años. El tipo de intervención más frecuente fue la linfadenectomía inguinal bilateral (44%). Solo cuatro pacientes fueron sometidos simultáneamente a penectomía y linfadenectomía. Las complicaciones posoperatorias se presentaron en 8 casos (44%), siendo el linfedema la más frecuente. Conclusiones: Los factores pronósticos adversos más importantes son la presencia de adenopatías, el estadío clínico al diagnóstico y el grado de diferenciación celular. El tratamiento más eficaz del cáncer de pene es quirúrgico, reservando la quimioterapia y la radioterapia para adyuvancia o terapia de rescate, siendo las complicaciones más graves, mientras más radical sea la cirugía.(AU)


Introduction: Penile cancer has a high incidence in Latin America, with squamous cell carcinoma being the most frequent histological type. It is also known that penile cancer has a spread to the inguinal lymph nodes, before the progression to metastatic disease. Objective: Characterize the patients treated for penile cancer with locoregional invasion in the Urology Service of the Hospital Central del Instituto de Previsión Social (HCIPS), Asunción of Paraguay. Materials and methods: A retrospective study was conducted of patients treated with penile cancer with locoregional invasion between 2014 and 2017. The following variables were analyzed: staging, treatments used according to the classification and complications. Results: The 18 patients treated for penile cancer with locoregional invasion were older adults, with an average age of 78.5 years. The most frequent type of intervention was the bilateral ilioinguinal (44%). Only four patients were subjected simultaneously to penectomy and lymphadenectomy. Postoperative complications occurred in 8 cases (44%), lymphedema being the most frequent. Conclusions: The most important adverse prognostic factors are the presence of adenopathies, the clinical stage at diagnosis and the degree of cell differentiation. The most effective treatment of penile cancer is surgical, reserving chemotherapy and radiotherapy for adjuvance or rescue therapy, being the complications more serious, the more radical the surgery is.(AU)

Among the malignancies, colorectal cancer ranks fourth in incidence in Brazil. The main prognostic measure is related to the amount of affected lymph nodes. Thus, many studies try to correlate the number of extracted lymph nodes, with the probability of obtaining positive nodes. Study objectives: Determine whether dissection ≥12 lymph nodes increases probability of finding neoplastic involvement in relation to resection of fewer. Assess the presence of angiolymphatic invasion; perineural and intracelluar mucin and correlate it with tumor differentiation and TNM classification. Correlate the average of positive nodes with angiolymphatic and perineural involvement. Methods: Pathological reports of patients operated for CRC from 1997 to 2013 were analyzed. A probability (p) less than 0.05 was considered to indicate statistical significance. Results: Median of lymph nodes sent to analysis was 12 nodes. Average number of lymph nodes affected was higher when a number ≥12 lymph nodes were dissected (p = 0.001) (Kruskal-Wallis). There was positive association between average of affected lymph nodes and presence of angiolymphatic (p < 0.0001) or perineural invasion (p = 0.024). Angiolymphatic and intracellular mucin are less present in well-differentiated adenocarcinomas. Perineural and angiolymphatic were more present in T4 stages. Conclusions: Dissection ≥ 12 lymph nodes increases chances of finding positive nodes. There is relation between angiolymphatic invasion; perineural and intracellular mucin and type of tumor differentiation, as well as TNM classification. Average number of lymph nodes affected was higher in presence of perineural or angiolymphatic invasion.


Dentre as neoplasias malignas, o câncer colorretal ocupa o quarto lugar em incidência no Brasil. Uma das principais medidas de prognóstico está relacionada à quantidade de linfonodos acometidos. Sendo assim, muitos trabalhos estudam meios de correlacionar o número de linfonodos dissecados, com a probabilidade de se obterem linfonodos positivos. Objetivos do estudo: Determinar se a dissecção ≥ 12 linfonodos aumenta a probabilidade de se encontrar acometimento neoplásico nos mesmos em relação à menor ressecção. Avaliar a presença de invasão angiolinfática; perineural e mucina intracelular e correlacioná-la com diferenciação tumoral e classificação TNM. Correlacionar a média de nodos positivos com acometimento angiolinfático e perineural. Métodos: Foram analisados laudos anatomopatológicos de pacientes operados por câncer colorretal (CCR) de 1997 a 2013. A probabilidade (p) menor que 0,05 foi considerada para indicar significância estatística. Resultados: A média de linfonodos comprometidos foi maior quando um número ≥ 12 linfonodos foram dissecados (p = 0,001) (Kruskal-Wallis). Houve associação positiva entre a média de linfonodos afetados e a presença de invasão angiolinfática (p < 0,0001) ou perineural (p = 0,024). A invasão angiolinfática e a mucina intracelular estavam menos presentes em adenocarcinomas bem diferenciados. Invasão perineural e angiolinfática estiveram mais presentes nos estádios T4. Conclusões: A dissecção ≥ 12 linfonodos aumenta as chances de se encontrar nodo positivo. Existe relação entre invasão angiolinfática; perineural e mucina intracelular e o tipo de diferenciação tumoral, bem como a classificação TNM. A média de linfonodos comprometidos foi maior na presença de invasão perineural ou angiolinfática.

ABSTRACT

ABSTRACT The concept of a hybrid operating room represents the union of a high-complexity surgical apparatus with state-of-the-art radiological tools (ultrasound, CT, fluoroscopy, or magnetic resonance imaging), in order to perform highly effective, minimally invasive procedures. Although the use of a hybrid operating room is well established in specialties such as neurosurgery and cardiovascular surgery, it has rarely been explored in thoracic surgery. Our objective was to discuss the possible applications of this technology in thoracic surgery, through the reporting of three cases.


RESUMO O conceito de sala híbrida traduz a união de um aparato cirúrgico de alta complexidade com recursos radiológicos de última geração (ultrassom, TC, radioscopia e/ou ressonância magnética), visando a realização de procedimentos minimamente invasivos e altamente eficazes. Apesar de bem estabelecido em outras especialidades, como neurocirurgia e cirurgia cardiovascular, o uso da sala hibrida ainda é pouco explorado na cirurgia torácica. Nosso objetivo foi discutir as aplicações e as possibilidades abertas por essa tecnologia na cirurgia torácica através do relato de três casos.
